Improvement of School Education in the Conditions of a New Stage of Development in the Republic of Uzbekistan | Author : Khakimov N. Kh | Abstract | Full Text | Abstract :The author of the article investigated the process of improving school education in a new stage of development of the Republic of Uzbekistan. The article emphasizes the role of school education in the implementation of democratic reforms during the implementation of the strategy of actions for the further development of the country. The author of the article made an attempt to reveal the mission of school education in a developing civil society. Topical issues of the educational process of secondary schools, in the upbringing of the younger generation and the practical significance of the activity of teachers in the context of modernization of the content and methods of teaching in secondary schools are investigated. An important innovative element in the work of secondary schools is that under the new conditions in the country there is a process of formation of private schools and an increase in the number of specialized schools, where certain fundamental subjects are studied according to a special, in-depth program. The country has made an attempt at an innovative approach in the process of improving school education and teaching children. In the context of the deepening development of the market economy in the country, the teaching staff performs an important mission in the implementation of social policy, ensuring the educational process in secondary schools, including a dynamic increase in the number of students participating in international Olympiads, in attracting students to the study of foreign languages. It should be noted that high school graduates are actively involved in deepening democratic reforms and ensuring human rights. A modern high school graduate is a powerful intellectual potential for the country, they make a worthy contribution to the future, the process of digitalization of society, the development of small business and private entrepreneurship. High school graduates pass a test selection and become university students in foreign, developed countries of the world. An integrated scientific approach to the study of the learning process in secondary schools shows that education in schools is closely linked with the goal of the ongoing democratic reforms in the country and the deepening of the market mechanism in all spheres of socio-economic life. It should be noted that the desire to improve the quality of the educational process in secondary schools has a potential impact on attracting talented young people to study at universities, technical schools, academic lyceums and professional colleges. In the process of school education, students are introduced to the knowledge necessary for the whole life of each person, which is considered a guideline for determining the future, finding a worthy place for a student in a competitive and globalized environment. The country has created equal legal conditions for high school graduates for admission to higher educational institutions. |
| Exploration and Analysis on Incidences of Covid-19 on the Senior Peoples and TCM Management-60 Clinical Cases | Author : Dan Jiang | Abstract | Full Text | Abstract :Incidence and prevalence of infected Covid-19 are still rampant in the West and the most countries throughout world from 2020 to 2021. Although the incidence of infected Covid-19 in the developed countries has decreased down by the popular vaccination, it is still anxiety and worry to make more serious widespread from the variated coronavirus. It is a rigorous fact that there still are higher on the mortality to the seniors, special seniors who are resided at nursing homes. For seniors, the infected Coronavirus may product them a disastrous attack according to their pre-existing diseases, complication accompanied with Covid-19, and post-syndromes caused by Covid-19. Their Covid-19 may be more complex, more serious, harder to find a fast and effective therapies and easier to become to the post-syndromes which will make a bad influence to the quality of life in the rest of their life.
TCM focuses to individual case, and pay attention more for their pre-existing diseases, complication with Covid-19, and possibility of post-syndrome’s production while some anti-coronavirus herbs are prescribed, so a more fixed prescription is made. In this clinical trial, the author classified 60 cases into two groups: over 45 years old as the observing group and under of 44 years old as the contrasted group, incidences on pre-existing diseases, complication and post-syndromes are observed separately; their results after treated by Chinese medical herbs are also observed. We recognize from these observation and analysis TCM treatment can significantly stabilized pre-existing diseases, treat complication together and prevent the post-syndromes’ occurrence while is treating to Coronavirus. TCM is a suitable treating choice to Senior with Covid-19. |
| Enterocutaneous fistula; Management Challenges,Areterospective Review of 11 Patients Treated in our Institute | Author : Aliya Ishaq | Abstract | Full Text | Abstract :Enterocutaneous fistula is a local pathology and systemic disorder.
OBJECTIVES:
To analyze postoperative outcomes, morbidity, and mortality in patients treated for enterocutaneous fistula in our institute for past 18 months.
DESIGN, SETINGS AND PATIENTS:
Reterospective review of records of patients presented to Liaquat national university hospital Karachi, Pakistan between Jan 2010 to June 2011 with diagnosis of EC fistula.
RESULTS:
A total of eleven patients presented with diagnosis of enterocutaneous fistula in our institute in specified duration. Mean age at presentation was 33 years with amle to female ratio of 3:1.72.7 % had high output fistula and 27.2 % had low output fistula. Small bowel was involved in 72.7%, large bowel in 18.18% and 9.0% had both small and large bowel fistula.45.45% patients had single fistula while 54.55 had multiple fistula. Total length of stay varied between 22-150 days .6/11 (54.54%) had nosocomial infection, 3/11(27.27%) had bed sores.2/11(18.18%) had TPN related complications.Spontaneous closure occurred in 8/11(72.77) patients and definitive surgical closure was performed in one patient. Mortality rate was 18.8 %.
CONCLUSION:
Enterocutaneous fistula is a devastating outcome for both surgeons and patients, sytemetic timely multidisciplinary approach can save lives. |
| Prevalence of Hepatitis B e-Antigen among Hepatitis B positive subjects in Nnamdi Azikiwe University Teaching Hospital Nnewi | Author : Onwurah O.W. | Abstract | Full Text | Abstract :Hepatitis B virus (HBV) is a common cause of liver disease throughout the world. Cirrhosis, liver failure and hepatocellular carcinoma develop in 15–40% of chronically infected hepatitis B virus individuals. HBV is transmitted through blood and other body fluids, including semen and saliva. The aim of this study was to determine the prevalence of HBeAg amongst HBsAg positive subjects in Nnamdi Azikiwe University Teaching Hospital, Nnewi (NAUTH) . This study was a cross sectional study and data were collected through oral interview and questionnaire. Five milliliters of blood was collected aseptically from the antecubital veins of consenting 100 HBsAg positive subjects into sterile containers free of anticoagulants. The sample was separated by low- speed centrifugation and the serum obtained was analyzed for HBsAg and HBeAg using diagnostic HBsAg panel kit (Diagnostic Automation/ Cortez Diagnostic Inc, USA). The result showed 6% HBeAg prevalence among the 100 HBsAg-positive patients in NAUTH. The HBsAg patients that tested positive for HBeAg were all males. The study concluded low prevalence of HBeAg among HBsAg patients in NAUTH. |
| Phyllodes Tumor of Breast:A 17 Years Audit of 260 Patients with Biopsy Proven Phyloldes Tumor Treated in our Institute | Author : Aliya Ishaq | Abstract | Full Text | Abstract :Phyllodes are rare breast neoplasms with higher rate of local and systemic recurrence. This study presents the results of an audit of all cases of biopsy proven phyllodes tumor presenting to breast services in Liquat National University Hospital Karachi, Pakistan, from 1993 to 2010. Total 260 female patients were retrospectively audited, 151 had benign, 35 borderline and 74 had malignant histology. Mean follow up duration for all types was 2 years. The mean age for benign lesions was 28, for borderline 36.8 and for malignant lesions 41 years. For all histopathological types the mean tumor size was 6.2 cm. It was 5.4 cm for benign, 7.8 cm for borderline and 11.2 cm for malignant lesions. 200/260 (76.9%) patients had breast conservation while 60/260 (23.0%) had mastectomy.
Histopathology type and grade has statistically significant relationship with recurrence ( p value .0001 % ) Overall recurrence rate was 20/260 (7.69%) with one patient with malignant phyllodes having recurrence 6 times despite clear margins and two patients with borderline phyllodes having recurrence twice . Tumor recurred in 2/151(1.3% ) of benign , 5.7% borderline ( 2/35 ) and 21.6% malignant ( 16/74 ) cases .Malignant group had 8 (10.8% ) systemic and 12 (16.2%)local recurrences. All patients with local recurrence and involved margins had reexcision except for 2 who had mastectomy with muscles involved posteriorly during the first surgery where re-excision was not possible. mortality rate for malignant phyllodes was 5.4 % ( 4/74 deaths as a result of systemic recurrence )
Conclusion: Phyllodes tumor is an aggressive disease, in our series, grade and type of tumor on histopathology is the significant risk factor for local and systemic recurrence with rate reaching up 21.6% for local and 10.8% for systemic recurrence in malignant lesions. |
